Gold Membership
As a Gold Plan Member, you’ll receive 100% coverage of all LiveWell services with the most comprehensive daily rate. This includes wellness services, care coordination, and care services at home, and if necessary, in an assisted living or nursing care facility.
Silver Membership
As a Silver Plan Member, you’ll receive 100% coverage of all LiveWell services up to a reduced daily rate. This includes services at home and if necessary, in an assisted living or nursing care facility.
Bronze Membership
As a Bronze Plan Member, you’ll receive all LiveWell wellness services, care coordination, and also home-based services up to a daily allowance. For each service, you’ll pay 50% of the cost. This structure includes services at home and if necessary, in an assisted living or nursing care facility.
Sterling Membership
As a Sterling Plan Member, you’ll receive all LiveWell wellness services, care coordination, and also home-based services up to a daily allowance. Assisted living or nursing care facilities are NOT included in this plan.
